This Website is for the use of our customers and the general public. The information presented regarding insurance in general or any specific coverage that may be discussed is accurate to the best of our ability. It must be understood that the actual insurance policy itself determines all coverage, definitions, terms, conditions, exclusions and provisions. Nothing in this Site can be construed to expand or constrict any insurance policy language or coverage.

This Site is to assist our customers and the general public to contact us in a convenient manor and to improve our ability to market our products. This Site CANNOT be used to bind any insurance coverage of any kind. Insurance coverage is only bound when you receive written notification form our office or directly form the insurance company.

Purpose Of This Notice

Title V of the Gramm-Leach-Baily Act (GLBA) generally prohibits any financial institution, directly or through it’s affiliates, from sharing nonpublic personal information about you with a nonaffiliated third party unless the institution provides you with a notice of it’s privacy policies and practices, such as the type of information that it collects about you and the categories of persons or entities of whom it may be disclosed. In compliance with the GLBA, we are providing you with this document, which notifies you of the privacy policies and practices of Carson, Smith & Associates.

Why We Collect Information

We collect the nonpublic personal information about you, your employees or your business that is necessary for our company to review, process or service your insurance requests. We gather this information in many ways including but not limited to the following sources:

• Information about transactions with our affiliates, others or us.
• Information received from a consumer agency.
• Information available form other sources such as government agencies.

 How We Disclose Information

This information we collect is used to make coverage, service, benefit and other insurance related decisions. This information is sometimes shared, as permitted by law, with our insurance affiliates and nonaffiliated third parties to carry out the daily business functions: review, process or service your products or services; or in connection with product offerings, claims or endorsements. Examples of nonaffiliated third parties are insurance companies, service administrators, mortgage companies, etc.

The Security We Provide

We take our customer’s privacy seriously. Carson, Smith & Associates restricts access to the information above to those employees who need to know the information in order to provide products or services to you or your employees.

This privacy policy notice is not in lieu of any other privacy notice issued by an other affiliate of Carson, Smith & Associates. It continues to apply even when your relationship with Carson, Smith & Associates has terminated.

Miscellaneous

These Terms are governed by and shall be construed in accordance with the laws of the State of Utah without giving effect to any principles of conflicts of law. If any provision of these Terms shall be unlawful, void, or for any reason unenforceable, then that provision shall be deemed severable form these Terms and shall not affect the validity and enforceability of any remaining provisions. These Terms are effective unless and until terminated by Carson, Smith & Associates.
